ESWL treatment option started in the early 1980’s and ESWL has now become the main therapeutic option for the majority of renal and ureteric stones. In some cases ESWL may also be used for the treatment of bladder stones.

International statistics show success rates of over 92% in the treatment of renal stones up to 2cm in diameter, but larger stones and those located in the lower pole of the kidney may fare badly and should be considered for an intervention. The management of ureteric stones has changed dramatically with the ESWL technology and all upper ureteric, most middle and some lower ureteric stones are now treatable with ESWL. Much of the success of ESWL for ureteric calculi is dependent upon immediate access to the service and in this situation ureteric stents can be avoided as it is possible to perform two treatments within a week and a success rates similar to ureteroscopy with a lower morbidity can be achieved.

Over the years this basic shock wave technology has undergone a number of modifications, but the basic concepts of shock wave treatment remain the same. The shock waves are high-energy amplitudes of pressure generated under water by sudden discharge of energy in a small reflector. This reflected energy propagates through water and soft tissues (in the form of shock wave) with little impact to the medium but exerts enormous energy when it encounters a barrier between substances of differing acoustic impedance (such as a stone). Repeated shock waves fragment the stone into small pieces which are then passed off in Urine. The treatment is usually performed without even analgesia but sometimes a simple dose of analgesic may be required, this procedure is mostly carried out in an outpatient setting.

All lithotripters are made up of four basic components; an energy source, a focusing device, a coupling medium and a stone localization system. But the basic universal target remains to generate shock waves with high-energy amplitudes of pressure.
